Position Summary:

The Associate Director of Acute Rehab Admissions and Discharge Planning oversees the admission processes and discharge planning functions for the acute inpatient rehabilitation unit. This role ensures seamless patient transitions, optimal patient flow, and compliance with regulatory standards, while leading a multidisciplinary team focused on patient-centered care. The Associate Director collaborates with physicians, case managers, and external referral sources to evaluate patient eligibility, coordinate admissions, and ensure timely and effective discharge planning.

Duties & Responsibilities:

Admissions Management

  • Oversee the pre-admission screening process to ensure patients meet medical necessity and rehab criteria.
  • Supervise the admissions team and manage daily referral activity, including outreach to referral sources.
  • Collaborate with physicians, payers, and case management to ensure appropriate patient placement and authorization.
  • Monitor admission KPIs and develop strategies to optimize occupancy and throughput.

Discharge Planning

  • Lead the discharge planning process in coordination with clinical staff, patients, and families.
  • Ensure comprehensive discharge plans are developed and executed efficiently, facilitating timely transitions to home or other care levels.
  • Establish relationships with post-acute care providers and community resources.
  • Regulatory documentation audits, ie: Important message, team conference, admission assessments.
  • Denial management

Leadership & Operations

  • Supervise and support admission coordinators, case managers, social workers, and discharge planners.
  • Provide coaching, performance evaluations, and professional development opportunities.
  • Develop and implement department policies, procedures, and workflows in line with hospital goals and regulatory requirements.
  • Assist with accreditation readiness (e.g., CARF, Joint Commission and CMS guidelines).

Collaboration & Communication

  • Serve as liaison between rehab services, acute care units, payers, and external providers.
  • Participate in multidisciplinary rounds and bed management meetings.
  • Ensure effective communication with patients and families throughout admission and discharge processes.

Working Conditions

  • Hospital-based environment with frequent interactions across departments.
  • May require extended hours or on-call responsibilities during peak admissions or discharges.
  • Performs other related duties as assigned.

Experience

5 to 7 years of related experience.

Education

  • Bachelors degree in related field required.
  • Masters degree preferred.

License Certification

  • A Valid Florida License is required: RN, or Therapist, or Social Work, or any other License in related field.
  • CRRN preferred.
  • Certified Rehab Admission Liaison preferred
