Salary: $42.21 - $52.76 per hour
Shifts: M-F 8:00am-4:00pm, with rotating weekends
Status: Temporary, Full time; 7 months with possibility of extension

Position Summary
The Myrna Daniels Seniors Emergency Medicine Centre aims to be a world leader in every domain of seniors' emergency care through building an innovative physical space, designing a transformative model of clinical care, seamlessly integrating with community partners, and producing leading-edge research and education. Seniors are the fastest growing demographic in Canada and require increasingly complex interventions resulting in an urgent need to develop, test and disseminate novel and sustainable approaches that can better meet the needs of an ageing population. The Centre is the first of its kind in Canada and aims to address these challenges through generating and translating new knowledge in the field of seniors' emergency medicine regionally and internationally.
The scope of SEM Centre encompasses the Toronto General ED, the Toronto Western ED and a future dedicated space for the Myrna Daniels Seniors Emergency Medicine Centre. Our aim is to ensure that all older adults can receive the same quality of care, regardless of site.
As an integral member of the interprofessional team, the Social Worker (RSW) provides services based on standards of quality and evidence-based practice.

Duties
- Assessing patients for the purpose of evaluating the need for service, the nature of the problem, and formulating recommendations or treatment plans
- Implementing an intervention plan with patients, family members, significant others, and the inter-professional team for the purpose of facilitating changes in behavior, attitudes, feelings and/or the environment
- Reassessing and modifying the intervention plan as required
- Liaising with appropriate community agencies
- Completing applications for Alternate Level of Care (ALC) facilities, establishing contact with members of the inter-professional team and community agencies to obtain, provide or exchange information relating to patients' psycho-social functioning and health care in the past, present and future
- Rendering services for the immediate benefit of the Hospital and the community
- Providing clinical teaching to Social Work Students, Medical Students, Professional Colleagues at University Health Network, Faculties of Social Work and in the community
- Performing activities required for the effective and efficient operation/maintenance of the Social Work professional group
- Contribute to quality improvement and development and implementation of a new model of care
- Provide social work subject matter expertise
- Work as part of a collaborative team that will include clinicians, quality improvement specialist, design specialist, data analyst, and more!
Qualifications
- Completion of a Masters Degree in Social Work (M.S.W) or equivalent
- Registration with the Ontario College of Social Workers and Social Service Workers
- Membership in the Ontario Association of Social Workers, preferred
- 2 - 3 years experience working with an interprofessional team in a health care setting, preferred
- Knowledge of policies, legislation, programs, issues related to social welfare institutions and services, the Health Care Consent Act, and Advance Care Planning
- Research experience /education, an asset
- Student education/supervision, an asset
- Knowledge of a second language, an asset
