The CARES for Kids Program Manager is responsible for leading the development, implementation, and oversight of a centralized care coordination model for Boston Children's Hospital's CARES for Kids Program. This role ensures the seamless integration of multidisciplinary care management strategies, collaborating with internal teams, ambulatory departments, and external partners to optimize patient outcomes. Serving as a clinical expert and strategic resource, the Program Manager provides guidance on care coordination processes, patient assessment, and intervention strategies. They also oversee program operations, including staffing, education, and performance, while fostering strong relationships with healthcare providers, behavioral health specialists, case managers, and community organizations.

They also oversee program operations, including staffing, education, and performance, while fostering strong relationships with healthcare providers, behavioral health specialists, case managers, and community organizations.
Principal Duties and Responsibilities
- Informing, implementing, overseeing, and evaluating a multi-pronged care coordination team for the BCH CARES for Kids Program
- Developing a strategic and operational plan with the team to guide the program's growth and implementation
- Establishing specific goals and metrics for the care management program and evaluating its outcomes on a scheduled basis
- Partnering with the leadership team to develop a centralized care coordination model, including defining roles, competencies, tools, and processes to enhance patient identification, assessment, intervention, documentation, and communication
- Providing clinical insight to support teams and collaborating with local ambulatory clinicians on nurse triage and care coordination needs for the CARES for Kids population
- Overseeing care management and coordination for CARES for Kids patients, ensuring alignment with best practices and patient-centered care strategies
- Supporting the initial and ongoing assessment of patients' and families' care management needs and self-management capabilities
- Building strong working relationships across the healthcare system
- Establishing consistent communication forums for internal and external stakeholders to enhance coordination
- Assisting the team in problem-solving highly complex situations and patient issues, serving as a clinical and operational resource
- Overseeing clinical and administrative direct reports and informing the staffing model needed for program execution
- Selecting, supervising, and evaluating clinical and support personnel
- Providing staff education and development programs
- Establishing consistent communication forums for staff
- Overseeing daily operations, including staffing levels, assignments, and deployment
- Assisting staff in solving highly complex situations and patient issues
To qualify, you must have :
- A current Massachusetts license as a Registered Nurse is required
- Bachelor's degree is required in the field of Nursing leadership, Program Management
- 5 years of direct patient care is required
- Work experience preferably in a pediatric management ambulatory and/or chronic disease setting is preferred
