The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, affect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

PROGRAM OVERVIEW

Transitions to Community Unit (T2C), located in the Bureau of Health Promotion of Justice Impacted Populations (BHPJIP) promotes community-based, public health cross-sector strategies that reduce criminal-legal involvement and improve the transition of individuals back to the community after incarceration and community supervision. T2C seeks to improve the physical, mental, and behavioral health and well-being of marginalized populations, with specific emphasis on persons involved and impacted by the criminal legal system. T2C trains and employs peer-led Community and Court-based Health Workers(or Court Navigators) who specialize in focusing on the role that healthcare and public health can play in addressing the complex health and social needs of persons with criminal legal system involvement.

POSITION OVERVIEW

This position sits within the Transition to Community (T2C) unit and expands on the New York City Health Justice Network (NYC HJN), an innovative program that aims to holistically improve the physical, behavioral, emotional health and wellbeing of persons impacted by the criminal legal system. The NYC HJN provides voluntary programming with the aim of increasing participants' access to, engagement in, and quality of community-based healthcare and social services. Supervisors will work both in court settings and community settings. Supervisors are expected to meet with other T2C mangers and staff at the Health Department regularly.

FPHNYC seeks a NYC HJN Community Health Worker(Court Navigator) Supervisor. Supervisors will seek to engage individuals as close to court involvement as possible with a trauma informed and strength-based approach. Supervisors will have oversight of both the Court-based Health Workers (Navigators), including supervision and guidance and CHW staff who work at HJN network partner sites including healthcare clinics and community-based organizations. Supervisors will also be part of a small team of leaders that meet regularly with Senior Program Managers to ensure all programming is provided in a timely effective manner.

RESPONSIBILITIES
  • Directly supervise and provide direction to Court- based Health Workers (Navigators) within the NYC HJN Court Navigator program.
  • Process referrals to the NYC HJN including checking eligibility of participants, assignment of cases based on participant's needs/preference, and monitoring of caseloads to assess capacity, volume, and productivity of Court-based Health Workers (Navigators).
  • Ensure all Navigator program services are documented in data management system as required by the Health Department.
  • Ensure timely and accurate internal reporting, including development and implementation of program reporting as needed to assess Navigator program effectiveness and monitoring of program outcomes.
  • Frequently visit sites to maintain relationships with key stakeholders and site champions.
  • Manage and maintain relationships with key staff at primary care clinics, court-based stakeholders and community-based organizations
  • Provide onboarding training and ongoing professional development, such as boundary setting, engagement skills, harm reduction, and other criminal legal involvement-based trainings.
  • Monitor key CHW/Court Navigator performance indicators, referrals, compliance to NYC HJN workflow, participant navigation and quality of service oversight
  • Work with the Community Advisory Board and participate with the Community of Practice with program partners.
  • Work with NYC HJN Data Analyst to track and monitor participant outcomes and maintain detailed, accurate and timely reports on productivity and quality.
  • Participate in the development, coordination of and facilitation of trainings for CHWs/Court Navigators, partner meetings and community events.
  • Assist with quality assurance and quality improvement efforts including conducting chart reviews and identifying opportunities for improvement.
  • Collaborate with HJN staff in the development, implementation, and iteration of HJN, CHW/Court Navigator policies, procedures, and programming.
  • Participate in strategic planning efforts and identifying areas for improvement.
  • Communicate with Senior Director and other senior program staff regularly to maintain program fidelity.
  • Complete required in-house trainings including, CHW Navigator Supervisor training and data platform training, and other trainings as required by the primary care clinics, court-based setting and community-based organizations.
  • If needed, maintain a small caseload of high-need, complex clients.
  • Provide operational coverage as needed, including traveling to different boroughs and providing vacation coverage as needed.
  • Perform other duties as assigned, including administrative project tasks.


QUALIFICATIONS
  • Master's degree in social sciences preferred (i.e., social work, psychology, public health, or related field) or a bachelor's degree in social work, sociology, psychology, public health, or a related field and 3-years Community Health Worker experience.
  • High school diploma or High School Equivalency (HSE) and extensive Community Health Worker experience including direct supervision may be substituted for formal education.
  • Demonstrated ability to plan, organize, and direct the work of others in a motivating and constructive manner.
  • Demonstrated effective leadership and supervisory experience.
  • Ability to work independently as well as collaborate, be flexible, and handle changing workload with multiple projects and changing priorities.
  • Experience providing counseling, outreach, workshop facilitation, case supervision and/or group/community presentations.
  • Ability to prepare, present, and review oral and written technical and confidential information and reports.
  • Strong relationship builder with excellent interpersonal and communication skills.

ADDITIONAL INFORMATION

Full Time Monday through Friday 9am -5pm

Location: NYC Court location and workspace Gotham Department of Health and Mental Health 42-09 28th Street, Long Island City, NY, 11101 and some travel between sites based on operational needs

RESIDENCY REQUIREMENT
