Description:
The staff physician shall work collaboratively with the MCHC health center team(s) to develop treatment plans and provide primary health care services and education to patients based upon their medical and psychosocial needs. Provision of services shall be in accordance with all applicable Federal and State laws, rules and regulations and in compliance with patient's Bill of Rights and shall be based upon evidence-based care standards.
Responsibilities
Provides quality medical care to patients in accordance with generally accepted standards of professional practice and the policies of the center.
Reviews all medical histories of patients upon initial visit to the center and maintains accurate documentation within the electronic medical record system.
Coordinates medical care for patients on his/her panel
Triages walk-in patient needs and calls or referrals received at the site and coordinates medical care.
Confers with the Chief Medical Officer for clinical/administrative supervision on a regular basis.
Collaborates with the health care team to ensure proper patient care and coordination. As a member of the Patient Care Team, attends and participates in daily huddles and practice team meetings.
Assists in the completion of patient care activities related to Patient Centered Medical home and other special programs, including ongoing care planning for high risk patient groups.
Participates in ongoing comprehensive care planning and review.
Provides onsite medical interventions when necessary and appropriate.
Refers patients to additional needed services within the MCHC Network or to an outside health care facility or health care practitioner for service(s) not available at the center.
Is at all times sensitive to cultural diversity in MCHC patient population and actively promotes sensitivity among staff on the care team.
Undertakes tasks and initiatives as assigned by the Chief Medical Officer and/or Chief Operating Officer.
Ensures prompt follow-up action on patients with abnormal test results or physical findings.
REQUIREMENTS
1. Holds a Medical Degree (MD) from a recognized medical school.
2. Currently licensed and registered as a physician with the New York State Education Department.
3. Board Certified or Board Eligible in a recognized medical specialty. BLS Certified

Pay Range: $170,000.00 - $190,000.00
