Key Responsibilities
- Complete timely clinical assessments and determine eligibility for case management services
- Develop and implement individualized care plans in collaboration with members, caregivers, providers, and support systems
- Conduct telephonic and in-person (home) visits to assess progress and evaluate care plan effectiveness
- Monitor ongoing member needs and update documentation to reflect interventions and outcomes
- Promote integration of medical, behavioral health, and long-term care services
- Facilitate interdisciplinary care team (ICT) meetings and collaborate with peers across disciplines
- Apply motivational interviewing techniques to support member engagement and goal achievement
- Identify and address barriers to care through coordination and resource linkage
- Travel up to 40% within assigned counties to perform in-person assessments
Required Qualifications
- Must reside in New Mexico - specifically in Bernalillo, Sandoval, or Valencia County
- Bachelor's degree in social work, psychology, public health, or a related field
(OR) at least 1 year of direct care coordination or case management experience - 1-3 years of experience in case management, disease management, managed care, or medical/behavioral health settings
- Valid driver's license with clean driving record and reliable transportation
- Must be available full-time, Monday-Friday from 8:00 AM to 5:00 PM MST

- RN licensure is not required for this position
- Behavioral health and social service backgrounds are highly valued
- This is a temp-to-perm opportunity offering $46/hr plus medical, dental, vision, and 401(k) with a 50% employer contribution towards premiums.
