Manager/Depression Care Program

Community Healthcare Network is seeking for a Full-Time Manager/Depression Care Program.

JOB FUNCTION:

Responsibilities include, but are not limited to:

• Identifies training opportunities, trains and supervises all program staff members
• After psychosocial assessment, develops indicated integrated care plans & provides/oversees provision of services
• Coordinates and facilitates communication between PCP and psychiatric consultant regarding patient care
• Oversees development of schedule of program in/out activities including evaluation of its effectiveness.
• Assists Depression Care Managers, social workers, Health Homes Care Managers, nutritionists and other providers with dually enrolled medical/behavioral health patients via case conferencing.
• Develops prevention case management and integrated behavioral health/primary care guidelines and procedures manual in accordance with SBIRT, IMPACT model and CHN's guidelines.
• Oversees recruitment, enrollment and retention of patients into behavioral health services.
• Act as crisis manager when situations required and/or when staff is absent or vacancies occur.
• Develops and maintains referral network.
• Oversees the planning of program schedules, scheduling of participants, preparing and distributing program announcements, preparing written program materials, and evaluating programs' effectiveness.
• Oversees data reporting for internal reports and for data submission to funding sources.
• Responsible for developing, implementing and reporting a comprehensive Quality Improvement Plan on a yearly basis with specific clinical indicators.
• Active participation on local, city and other behavioral health related networks.
• Develop, with the VP of Social Services and Director of Behavioral Health Services, white papers to be presented at local, state and national conferences representing Community Healthcare Network.

WHAT WE LOOK FOR:
Licensed Clinical Social Worker preferred with at least three (3) years of experience working in behavioral health field as well as one (1) year of management experience.
