Job Summary:
The Acute Care Nurse Practitioner will practice within the Southeast Regional Stroke Center under the supervision of Medical Director. Responsible for providing direct and/or indirect patient care and supervising the care of patient who are admitted to and consulted on by the Neurology Service Line. Provides direct patient care for patients diagnosed with various neurological conditions, including but not limited to acute ischemic stroke, ICH, Seizure disorders. Responsible for prompt recognition of acute change in patient status necessitating an emergent change in the plan of care. Responsible for responding to all CODE STROKES called from the Emergency department or in-house.
Patient care includes daily rounds independent of physicians, performing detailed assessments, and treating patients with current scope of care, collaborating and consulting multidisciplinary team to optimize patient care and outcomes. Will work closely with the ancillary departments to ensure appropriate and timely work up and disposition of all patients. Provide a comprehensive discharge plan to facilitate proper transition of care. Serves as both role model and resource to nursing staff. Provides ongoing staff and patient education as needed. Provides patient education on ongoing basis. Maintain current credentialing/privileges, CEU requirements, licensure and certification as required. Serves as a liaison between physicians and staff.
The Practitioner will practice within the field of Neurological medicine under the supervision of Medical Director. The Practitioner will work in collaboration with the Neurologists of the Southeast Regional Stroke Center in providing healthcare services to patients in the inpatient and outpatient setting. Range of services is set forth in a written protocol developed jointly and signed by the Practitioner and the supervising physician. General activities will be in the evaluation and treatment of patients for neurological conditions. Practitioner will be involved in all aspects of the practice including but not limited to: patient 90-day follow up visits post discharge, inpatient consultation and follow up, and assistance with call coverage for 24/7/365 in-house neurology services. Practitioners are responsible for working a 7 days on/ 7 days off schedule to include weekends and holidays. Practitioner must have the ability to tailor specific initiatives for Neurology patients. Must interact with employees and learn their specific needs to help with the safety and health of the patients served. Must uphold high ethical standards and practice within the scope of practice set forth in the written protocol. Complete records in a timely manner.
The Nurse Practitioner supports the delivery of evidence based acute stroke assessment and management, provides staff education such as Neurology Anatomy and Physiology, Gold Standard for stroke; Get With The Guidelines and Joint Commission Requirements, Neurology assessment, stroke mimics, stroke syndromes, pre-hospital care, stroke management in the Emergency Department, diagnostic tests, stroke protocol and the care of the stroke patient, stroke rehabilitation; criteria for transition of care, home health and community based care, and the role of the caregiver. The Nurse Practitioner must be available for consultation regarding the care of the patient experiencing an acute stroke. The Nurse Practitioner will serve on the following committees:
-Service Line Committee
-Process Improvement Committee
-M & M Committee
The Nurse Practitioner will participate in research for improving outcomes for the stroke patient, assist in community outreach for stroke education and education of the patient and caregiver for the stroke patient.
This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines. Is trained and assigned (as per their role and responsibilities) to coordinate care for patients and is trained and assigned to support patients/families/caregivers in self-management, self-efficacy and behavior change. Is trained and assigned (as per their role and responsibilities) to manage the practices patient population. Participates in the practice quality improvement process and performance evaluation.
Education:
Required:
Masters of Science Degree in Nursing (MSN)
Successful completion of an accredited Acute Care Nurse Practitioner Program.

Experience:
Required:
Minimum of two (2) years Nursing clinical experience required.
Preferred:
Neurology experience preferred.
Position Requirement(s): License/Certification/Registration
Required:
Licensed to practice nursing in the State of Tennessee,
Licensed as an Advanced Practice Nurse (APN) in the State of Tennessee.
BLS completed prior to going to the unit/ department and maintained going forward
ACLS Certification within 30 days of hire
8 hours of stroke education annually
The Nurse Licensure Compact will not change how to obtain or renew a Tennessee license. However, the Tennessee nursing license will be a single state license for Tennessee Residents or non-compact state residents. Tennessee licensure or multistate licensure from a compact state must be obtained within three months of hire for non-Tennessee residents.
