License: Registered with the Board of Registration in Massachusetts

Must-Have:
- Two years of experience in acute care, rehab, or homecare as a Speech Therapist
- Proficient computer skills with the ability to learn new software applications
- Experience working in an interdisciplinary team
- Strong verbal, written, and electronic communication skills
- Excellent interpersonal and customer service skills with the ability to work with a diverse population
Description:
Provide speech therapy services to patients in their place of residence.
Patients may range in age from birth to over 100 years.
Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate services.
Manage technical, environmental, and financial resources.
Key Responsibilities:
Complete age-appropriate evaluations reflecting patient history, relevant systems review, and appropriate tests and measures.
Report findings to the primary clinician.
Reassess the plan of treatment at least every two weeks and communicate changes to the patient, primary caregivers, and interdisciplinary team.
Ensure physician orders are current and consistent with the plan of care and documented in the medical record.
Submit patient schedules to the rehab scheduler by the end of day Thursday.
Orient and supervise home health aides in accordance with agency and state guidelines.
Manage patient caseloads according to agency, state, and professional guidelines.
Complete and export documentation timely, adhering to agency standards.
Update POC devices weekly and as directed.
Complete verification of payor information on admission/evaluation within regulatory guidelines.
Maintain documentation to support managed care authorizations and DME requirements.
Ensure visit schedules are consistent with authorizations by managed care payors.
Discuss utilization and caseload with the rehab manager on an ongoing basis to meet stated goals.
Identify realistic, objective, and time-oriented goals for patient performance based on examination findings.
Select treatment and discharge plans based on examination findings and patient goals.
Inform patients of their weekly schedule and any changes as necessary.
Achieve reportable outcomes that meet agency goals.
