Job Description
Job Title: Home Health Physical Therapist
Profession: Therapy/Rehabilitation
Specialty: Home Health
Duration: 13 weeks
Shift: Day
Hours per Shift: 8 hours (08:00 - 16:30)
Experience: 1 year of recent Skilled Home Health Experience (at least 1 year within the last 3 years)
License: Must have a valid state license or compact license
Certifications: BCLS
Must-Have:
All clinicians will be required to start after May 5th.
Please ensure this expectation is communicated to clinicians prior to submission.
Home Health PT position available for ongoing patient needs.
Shifts consist of 8-hour days.
Ratios are 1:1 with the possibility of visiting up to 6 patients per day.
A productivity point standard of 30 is in place.
First-timers are accepted.
Weekend on-call requirements include every fourth weekend and a weekday on-call 2-3 times per month.
Local candidates are welcome.
Travelers are expected to have their own reliable transportation.
The system effectively groups clients to minimize drive and windshield time.
There may be an occasional maximum radius of 50+ miles for visits.
Timely communication and documentation are essential.
Documentation tools will be provided and must be returned.
Additional Notes:
Travelers are expected to float between all facilities within a given state.
Refusal to float may result in termination of the assignment.
All travelers should have prior experience with specified charting systems.
Modules are non-billable and average 16 hours of completion time.
Completion of modules is self-reported, with results recorded internally.
Orientation consists of 40 non-billable hours.
