Job Description
Job Title: Home Health Physical Therapist
Profession: Therapy/Rehabilitation
Specialty: Physical Therapy Home Health
Duration: 13 weeks
Shift: Day
Hours per Shift: 8 hours (08:00 - 16:30)
Experience: 1 year of recent Skilled Home Health Experience
License: Must have a valid state license or compact license
Certifications: BCLS
Must-Have: Reliable transportation
Description:
All clinicians will be required to start after the specified date.
Home Health Physical Therapist needed for Everett and surrounding areas.
The start date is as soon as possible, with compliance taking about two weeks.
Shift is 8-hour days with a ratio of 1:1.
Clinicians may visit up to 6 patients per day.
Productivity standard is 30 points.
First-timers are accepted.
On-call requirement includes every 4th weekend and a weekday on-call 2-3 times per month.
Local candidates are accepted.
Timely communication and documentation are required.
Documentation tools will be provided and must be returned.
Travelers are expected to have their own transportation.
The system effectively groups clients to minimize drive and windshield time.
Occasional maximum radius of travel may exceed 50 miles.
Experience with Oasis is required.
Travelers should provide supervisory references covering one year of employment within the last three years in a comparable specialty or setting.
All travelers are expected to float between facilities within a given state.
Refusal to float may result in assignment termination.
Providers must complete orientation for 40 hours as a non-billable requirement.
Modules are non-billable, with an average of 16 hours spent before the assignment and annually.
Time spent on modules is self-reported.
