Responsibilities
Serves as the ambulatory care pharmacy team leader, developing and maintaining clinical expertise. Incumbents function as the department representative for Pharmaceutical Services in ambulatory care, and participates in departmental and interdisciplinary programs directed at improving patient care standards. Provides education, mentoring, and coaching for staff development, residents, students and interns for the service line. Provides direction and solves problems for ambulatory care pharmacy and communicates actions with department managers. May serve as the Residency Program Director or Coordinator if applicable to their location.
Incumbents are also accountable for keeping professional skills current; developing and maintaining professional contacts with appropriate community resources; participating in continuous performance improvement; assisting in the orientation of new staff,the supervision of students and volunteers, and consistently making efforts to achieve extraordinary results.
Qualifications
Required Education and Experience
Pharm.D. degree
Clinical experience with 3 years of recent direct patient care in the ambulatory care setting
Required Licensure and Certifications
Active State Pharmacist License in good standing in the state in which the employee works: PHARM:XX
Required Minimum Knowledge, Skills, Abilities and Training Current knowledge of evidence based clinical pharmacy practice and pharmacotherapies
Knowledge of Health Plans such as Medicare, Medicaid, and commercial plans
Demonstrates knowledge and skills necessary to provide care appropriate to all age groups of patients served and infection control, safety and risk management standards as relates to the ambulatory pharmacy setting.
Knowledge of the practices and techniques relating to staff education and development.
Ability to effectively design and coordinate evaluative studies and research-oriented investigations.
