DSHS ALTSA Nursing Care Consultant - Nurse Community Consultant

TheAging and Long-Term Support Administration,Home and Community Serviceshas an opportunity for a Nurse Community Consultant located in King County, WA.In this role, you'll be part of our hardworking team of professionalsthat provide services to vulnerable adult populations. This position is at the specialist level and independently provides intensive and expert services to a population of vulnerable, physically and cognitively impaired, and frail adults in the community who are medically and financially dependent on Medicaid and in need of any of the full range of services.

Some of what you'll do:

• Complete nursing assessments of applicants including those clients with complex medical problems, for appropriate services.
• Provide nursing consultation to the client, family members, health care providers and other professional to develop an appropriate plan of care. Also provides consultation to staff, which may include joint field visits to develop an appropriate plan of care.
• Conduct comprehensive assessments and care according to minimum standards.
• Develop initial and ongoing care plans per minimum standards which provides the support necessary to allow clients to remain in or return to the community from nursing facilities, hospitals, or oother institutional care settings; or permits an alternative to nursing home placement through our programs.
• Provide nursing case management services for private duty nursing clients, including development of a service plan, implementation of that plan, service plan review and termination.
• Provide client advocacy, as well as consultation, networking, family support and crisis intervention.

Desired Skills & Abilities:

• Knowledge of community resources related to elders, mental health, medical care & supportive services.
• Must be able to communicate and comprehend (through oral and written forms of communication) with management, peers, clients, and the public.
• Effectively communicate and educate staff, clients and caregivers, regarding medical information, nursing care issues and the disease process.
• Ability to apply Social casework theory, principles and practices.
• Understand the physiological disease process in relation to skilled nursing treatments for preventative health and maintenance.
• Ability to critically review complex medical information for analytical review for further assessment, care, treatment planning and to assess for level of risk.
• Identify and evaluate social dynamics affecting client choice and decision making.
• Demonstrate flexibility by adapting to change, respecting differences in opinion and values.

Professionals with one of the following should apply:

• A Bachelors degree involving major study in nursing and three years of clinical or functional nursing experience.
• A Masters degree in nursing may be substituted for one year of clinical or functional nursing experience.
• A valid Washington State license to practice as a Registered Nurse (RN) is required.
