Job Description:
Are you passionate about making a difference in healthcare? Join our team as a Clinical Manager - Integrated Care and play a vital role in transforming patient transitions from hospital to home. Help reduce hospital readmissions and improve health outcomes by providing direct care, participant education, and coordinating with interdisciplinary teams (IDT). Supervise home care staff, ensure compliance with clinical standards, and deliver high-quality care at home. Lead care transition planning and execution, collaborating with hospital discharge planners, physicians, and home care staff to create personalized care plans that align with organizational goals and best practices.

Work Environment:This role operates within an onsite medical clinic and community healthcare setting, including locations in Westchester. Physical requirements include standing for up to 6 hours a day, sitting for 6-8 hours consecutively, and lifting/pushing/pulling up to 50 pounds. The role also involves bending/squatting, maneuvering stairs, walking between sites, and demonstrating agility and fine motor skills. Visual and auditory acuity is necessary for accurate patient assessment and intake processing. Cognitive abilities for decision-making and rational processing are essential. The work schedule is from 8:30 AM to 5:30 PM, totaling 40 hours per week.
Pay and Benefits
The pay range for this position is $135000.00 - $140000.00/yr.

Lead care transition planning and execution, collaborating with hospital discharge planners, physicians, and home care staff to create personalized care plans that align with organizational goals and best practices.
Responsibilities:
- Develop personalized care transition plans in collaboration with hospital discharge planners, physicians, and interdisciplinary teams.
- Review medical histories, discharge instructions, medications, and follow-up care to ensure smooth, safe transitions.
- Educate participants on post-discharge care plans, including medication management, wound care, and follow-up appointments.
- Conduct comprehensive home visits to assess health, living environment, and support systems.
- Monitor participants' conditions, identifying changes or complications that require care plan adjustments.
- Work with the IDT to ensure comprehensive care coordination and optimize participant outcomes.
Essential Skills:
- Active and unrestricted license as a Registered Nurse in New York State.
- Degree from an NLN-accredited nursing school (RN, BSN preferred).
- Minimum of 3 years of clinical nursing experience, including 1-2 years in home care or similar healthcare settings.
- Experience in care transitions, case management, or PACE programs preferred.
- Strong clinical assessment and critical thinking abilities.
- Excellent communication skills.
- Ability to manage multiple participant cases effectively.
Additional Skills & Qualifications:
- Bilingual preferred.
- Availability for weekends/nights coverage as needed.
- Ability to travel to participants' homes and work in varying environmental conditions.
- Proficiency with computers and electronic health record (EHR) systems.
- Experience with long-term care or managed care systems beneficial.
- Vehicle and current New York State driver's license preferred.
