POSITION SUMMARY
Under the supervision of the Nursing Manager/Director of Nursing, the Registered Nurse (RN) is responsible for the overall oversight and supervision of the healthcare needs of the people we support. The RN will work in conjunction with medical providers to ensure the delivery of the highest quality of integrated health and continuity of care to the people supported across the FREE network.
DUTIES, RESPONSIBILITIES, AND ESSENTIAL FUNCTIONS
Reviews and updates the individual's plan of nursing care annually or as needed in accordance with medical diagnoses and treatment plan; ensures staff are trained on plan of nursing care.
Conducts spot checks on medication counts, ensures accuracy of narcotic and needle/syringe records, verifies adequate supplies of medication and first aid materials, and ensures proper storage, disposal, and compliance with agency policy through monthly medication storage counts.
Validates the Medication Administration Record (MAR) on a weekly basis for required information such as medication given as ordered and started in a timely fashion, no blank electronic signatures, PRN symptoms are charted and result stated, note reason for any discontinued medications; validates paper MARs as required when is use.
Checks that doctor's orders and prescriptions are current, medication formularies are in
place for each medication, and MARs completed correctly; prints MARs as needed if a paper MAR is being used.
Ensures Grab and Go books are current for use during appointments or hospitalizations.
Ensures that procedures for new medication orders are accurately completed.
Checks that all medication administered has a matching label, order, and medication sheet entry.
Monitors effects of medication; documents as needed.
Tracks and documents all medical appointments, illness or accidents, current medications,
medication changes; current diet (weight loss and gain), and all charts required by the
individual's nursing care.
Assists with ensuring that all physician appointments and follow ups are scheduled as ordered; reviews incoming physician summaries for instructions, medication changes and treatment orders and implements all changes.
Ensures that all emergency room/hospital discharge instructions are followed up and documented.
Tracks all annual appointments for assigned caseload to ensure that required appointments are scheduled and attended.
Completes all necessary nursing assessments per agency policy (quarterly, bi-annually,
annually, as needed), including, but not limited to, as Fall Risk assessment, Nursing Visuals, Self-Administration of Medication Evaluation, and Tardive Dyskinesia Screening, and Braden Scale.
Performs physical observation of individuals (skin/nails/hair/overall health).
Reviews incident reports and occurrences; counsels and/or re-train staff as needed to ensure
accurate reports.
Validates accuracy and timeliness of entries in all required individual's charts.
Conducts Nursing In-service training for residential staff on topics including but not limited to recognizing illness, injury, or disease, taking vital signs, addressing health needs, administering treatment, performing first aid, following emergency procedures, and writing incident reports.
Maintains a record of dates when required in-service sessions for residential staff were conducted.
Assists with AMAP certification/recertification by observing and documenting med pours to ensure competency of skills and providing AMAP refresher courses to staff.
Participates in meetings regarding the care and concerns of the individuals, including
ISP's, Interdisciplinary Team Meetings, Project Management meetings, Nursing/Management meetings, and House meetings.
Maintains communication with physicians, day programs, and family members regarding
the individual's care.
Ensures that program staff maintain a communication log for each other to inform residential program and nursing staff as appropriate.
Reports possible violations of individual's rights promptly, and completes and submits
incident reports in accordance with New York State regulations and agency policy.
Reports ongoing problems and suggest solutions to improve working/environment
conditions.
Handles program equipment, consumable materials, and resources in a cost-conscious
manner.
Keeps updated and reviews any regulation changes in State or Federal policy and/or
agency procedures.
Provides projected weekly schedule to the Nurse Manager/Director of Nursing.
If position includes being "on call," ensure timely response to calls from the residence,
day program, family members, etc.
Attends and completes training/certifications in accordance with New York State regulations and agency policy, including, but not limited to OPWDD training and SMLS training.
Maintain copy of your current RN license with the Human Resources Department.
Accurately and positively communicates agency philosophy and administrative decisions
to other staff in a timely and professional manner.
Assists in maintaining agency property in a neat, orderly, and safe manner and reports all
repair needs in a timely manner as per site-specific needs.
Follows agency policy and procedures for building security and identification of
authorized personnel.
Follows HIPAA and OSHA regulations, including universal precautions.
Checks that the following are posted and used correctly: Poison Control number,
emergency numbers, pharmacy telephone numbers, medical providers, and nursing on call
information.
Provides transportation as required.
Reports all violations and/or accidents in agency vehicle in accordance with New York State regulations and agency policy.
Addresses emergency situations as they occur in the program according to established
policies and procedures, and ensures communication of such situations in a timely manner.
Provides coverage in emergency situations until relieved of the responsibility.
Ensures the physical safety and security of the individual by taking precautionary and
preventative measures against conditions in the environment which might cause physical
harm.
Ensures protective and preventive measures for staff and consumers are followed to control communicable and infectious diseases, including identification, assessment, and reporting.
Ensures that the individual's needs are met and intercede on the individual's behalf as
necessary.
Treats each individual with dignity and respect in accordance with New York State
regulations and agency policy.
Utilizes information acquired through training, meetings, education, and experiences to
benefit individuals.
Ensures that all required documentation (paper and electronic) is completed /processed appropriately, in a timely manner, and meets quality and compliance thresholds.
Performs other duties as assigned which can include supporting alternate locations as needed.

QUALIFICATIONS
Education:
o Associates degree in Nursing required; Bachelors degree in Nursing preferred
Experience:
o Experience working with the ID/DD population preferred
Specialized Training, Licenses, Certifications:
o Valid NYS Licensed Registered Nurse
Knowledge, Skills, Competencies, and Abilities:
o Knowledge of OPWDD, DOH, and OMH state and federal regulations preferred.
o Ability to manage multiple priorities and ability to be flexible to meet the needs of a
diverse population in a variety of settings.
o Strong verbal and written communication and organizational skills.
Other Requirements:
o Valid Drivers license and able to satisfy criteria to be an agency driver
o Able to be on call as needed
WORK ENVIRONMENT
This role is performed in-person; some travel required.
PHYSICAL REQUIREMENTS
Employee is required to sit, walk and stand for prolonged period of time (at least 8 hours).
Employee is required to effectively communicate both verbally and in writing.
Employee is required to operate standard office equipment such as computers, phones, photocopiers, fax and filing cabinets.
Employee is required to reach with hands and arms.
Employee is required to bend, stoop, kneel, crouch or stand on a stool as needed.
Employee is required to be able to lift 50 pounds at a time.
