- Conduct interviews and perform initial client assessments
- Manage a caseload of social service recipients
- Make home visits to assess the needs of adult clients
- Investigate and verify client information for accuracy
- Provide health-related and social services support
- Collaborate with interdisciplinary teams to ensure comprehensive care
Requirements:
- Experience: Minimum 1 year in a social services or case management role
- Education: Bachelor's Degree required
- Skills:
- Strong understanding of social services processes
- Excellent interpersonal, communication, and assessment skills
- Languages: Proficiency in English (Speak, Read, Write)
