Description of Work

The Adult Services Department of the Wilson County Department of Social Services provides services to individuals over the age of eighteen. These individuals have a wide range of needs, lifestyles, and handicaps. They may be physically and/or mentally handicapped, or they may have no medical problem at all. The primary purpose of this position is to serve as the nurse for the In Home Aide Program. This includes providing supervision for In Home Aides, completing assessments for customers receiving In Home Aide services, and ensuring all State, County and agency policies are followed accurately. In-Home Aide services are intended to assist individuals, elderly and disabled and their families in attaining self-sufficiency and improving the quality of life. The long term goal of In-Home services is to allow the client to remain in their home as long as possible, within safe living conditions. Aide services attempt to prevent deterioration of an individuals' functional capacity and maintain family functioning as a unit. In-Home Aide services also work to prevent and/or improve conditions of abuse, neglect, exploitation, and dependency. Prevention of out of home placement is also another main purpose of In-Home Aide services, as well as assisting the client to readjust to returning home after residing in a Nursing home or Rehabilitative facility. The nurse will assist families in caring for their elderly/disabled members within the environment of their own homes, not to replace the care and support these families offer.
Description of responsibilities and duties include but are not limited to:
  • Complete interviews/assessments with aides that desire to work in the program.
  • Supervise aides working in the program to ensure quality services are being provided.
  • Provide ongoing training for aides on various topics
  • Complete initial and ongoing assessments of customers receiving In Home Aide Services.
  • Complete an In Home Aide Service Plan with each customer that includes measurable outcome goals, specific tasks to be
    performed, frequency of service provision, duration of service, and conditions of continuation or discontinuation of services
  • Establish effective working relationships with community partners to gain support in meeting the needs of In-Home Aide
    customers.

Employee must possess valid driver's license and dependable transportation.
Minimum Education and Experience

Graduation from a four-year college or university with a B.S. in Nursing which includes a Public Health Nursing rotation and one year of Public Health Nursing experience; or Master's in Public Health and graduation from a school of professional nursing and one year of professional nursing experience; or graduation from a school of professional nursing and two years of professional nursing experience including one year of Public Health Nursing experience; or an equivalent combination of training and experience. Necessary Special Qualifications - A current license to practice as a Registered Nurse in North Carolina by the N. C. Board of Nursing.
