The Department of Family and Community Medicine at Thomas Jefferson University is seeking a new Ambulatory Palliative Care Provider!

Job Description

Responsibilities:
  • Directing and providing supervision for RN's who will be managing patient care
  • Provide supervision for RNs who do home visits
  • Deliver longitudinal care for patients through ambulatory, telehealth, and home-based palliative care services within the Department of Family and Community Medicine.
  • Provide expert-level care in collaboration with an interdisciplinary team (including SW, RN, APP, PharmD, etc.) to address the physical, psychological, spiritual, and emotional needs of patients with serious illness.
  • Facilitate the clarification and development of patient and family goals of care.
  • Improve awareness of and access to palliative care services among patients, families, and clinicians.
  • Collaborate with inpatient palliative care consultative services to ensure smooth transitions of patient care
  • Collaborate with and provide supervision to advance practice clinicians (NP/PA)
  • Responsible for coordinating with population health for palliative care initiatives
  • Provide teaching and mentoring to rotating medical students, residents, and fellows in the Jefferson system
  • Collaborate with specialists (oncologists, pulmonologists, etc) and primary care physicians to ensure optimal patient care
  • Exhibit expert-level, compassionate bedside manners and communication skills.
  • Collaborate with other team members to promote quality and process improvement and program development
  • Exhibit competency using the EMR effectively and efficiently
  • Call will be shared by other physicians or APPs in the group or enterprise-wide if the first clinician at a site


Qualifications:
  • Graduate of an accredited MD or DO program
  • Completed accredited residency program (IM, FM preferred for outpatient positions)
  • BC/BE in Hospice and Palliative Medicine, Fellowship preferred
  • Current medical license is in good standing in Pennsylvania (or NJ for NJ positions)
  • DEA license
  • Demonstrated ability to work effectively in a matrixed environment
  • Demonstrated commitment to diversity, equity, and inclusion
