Job Description: Under the leadership and direction of the Director - Five Hills Access Centre, the incumbent is responsible to act as the primary liaison between physicians, facility nursing unit managers, facility health care providers, and Continuing Care services (Home Care, Transition Programs - respite, waiting placement, convalescence, palliative care, long term care, Geriatric Assessment and Rehabilitation Unit, Adult Day Program) The Assessor Coordinator facilitates the transition/ discharge planning process from the acute care facility back to the community or Long Term Care in accordance with SHA policies, procedures, and guidelines.

Education
- Bachelor of Science in Nursing (BScN)
Licenses
- Valid Class 5 driver's license
Other Education and Training
- Psychiatric Nursing diploma is acceptable
- Bachelor of Psychiatric Nursing is acceptable
- Bachelor of Social Work is acceptable
- Nursing diploma is acceptable
