LSSI is hiring a full time Registered Nurse, Monday through Friday from 9:00am to 5:00pm. Assertive Community Treatment (ACT) team on the Northwest side of Chicago. ACT Services consist of integrated crisis, treatment and rehabilitative supports provided by an interdisciplinary team to individuals with serious and persistent mental illness or co-occurring mental health and substance use disorders. ACT Services are intended to promote symptom stability, management of co-morbid health conditions, and appropriate use of psychotropic medications, as well as to restore personal care, community living, work and social skills. ACT Services encompass counseling and therapy, medication management and monitoring, skill building, and crisis stabilization services. ACT Services focus on the restoration of functional skills (e.g., psychosocial, adaptive, self-care) to promote and maintain community living.

The Registered Nurse provides direct nursing services to all enrolled clients and works with the team to monitor each client's clinical status and response to treatment. The registered nurse functions as a primary practitioner on each team's caseload of clients. They can play a significant role in screening and care management of co-occurring medical issues.

Essential Functions:
  • Provide health promotion education to clients and guidance to staff.
  • Operate as part of a treatment team and attend daily staff meetings.
  • Medically screen clients to provide disease management in accordance with the psychiatrist/APN/medical physicians.
  • Provide direct nursing services to clients for health history, weight, BMI, etc. in order to safely facilitate their transition into independent living.
  • Coordinate health information exchange with psychiatrist/APN/medical physicians/MCOs in accordance with privacy laws.
  • Comply with documentation and productivity standards.
  • Transportation of clients in personal vehicle and agency vehicle required. Services will be provided at multiple locations.
  • Ensure all documentation is completed in a timely manner and meets Rule 140/132, agency/regulatory, and/or MCO requirements. This includes crisis plan assessments and intake documentation.

Position Qualifications:
  • Medical degree, Master of Science in Nursing, or Master's degree from an accredited institution.
  • RN required & proven nursing skills.
  • 1 year of experience in behavioral health or psychiatric setting, preferably in a community-based setting preferred.
  • Must be willing to work with clients in their home/community.
  • Bilingual Spanish/English preferred.
  • Excellent communication, organization, and PC/computer skills (including proficiency with Microsoft Office - Outlook, Word, Excel and PowerPoint) along with other related software.
  • Strong organizational skills, attention-to-detail, ability to prioritize tasks, and manage time effectively.
  • Demonstrated ability to interact with and respect differences of opinion, beliefs, culture, appearance and ways of life.
  • Ability to problem-solve and make decisions in collaboration with the client.
  • Ability to establish trusting, hopeful relationships with clients and their families.
  • Driving is required.
  • Access to reliable transportation is required.
  • Valid Driver's License in good standing for the state of residency and insurance.
