The Cognitive Care Network Community Navigator is responsible for developing relationships with health care providers (primarily MD, DO, APRN) in designated Kansas counties and, in collaboration with the Director, advance the use of clinically indicated cognitive screening tools, enlist providers in the Cognitive Care Network and advance research referrals. Once a provider is part of the Cognitive Care Network, the Community Navigator will serve as Care Navigator for patients referred from those practices for post diagnosis education and support as well addressing the needs of patients with care complications. The Community Navigator must possess advanced understanding of Alzheimer's and other dementias, assessment skills, interventions for dementia associated neuropsychiatric challenges, understanding of the activities of the center, including clinical services, research, and outreach, as well as understand the demands and challenges of provider practices.

The Community Navigator will provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education, and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.Job Description:

Job Responsibilities:
  • Establish and maintain positive relationships with providers and continually evaluate barriers to practice translation of cognitive health and dementia care protocols, Cognitive Care Network participation and integration of research connections.
  • Use clinical knowledge and background to inform and coach providers in use of cognitive screening tools and clinical application to specific patient populations.
  • Track number of provider contacts/status, as well as referral data tracking.
  • Use clinical knowledge to counsel and educate patients and families post diagnosis disclosure.
  • Use clinical knowledge to assess contributors to neuropsychiatric challenges associated with dementia and to make recommendations to both family and clinical team.
  • Use clinical knowledge to develop, along with patients and families, dementia management plans.
  • Travel required, primarily to locations in rural Kansas, to conduct onsite visits at assigned network practices, engage with patients and their families, collaborate with practice providers and staff, and participate in community outreach activities.

Required Qualifications

Education: Master of Social Work or related degree/clinical licensure. Education may be substituted for experience on a year for year basis. Experience used to substitute education is in addition to any required work experience.

Work Experience:
  • 6 months of experience working in dementia care including: direct work with individuals identifying with memory concerns, individuals diagnosed with a dementia and families of those diagnosed.
  • 6 months of experience in clinical research or community outreach.
  • Available to work occasional evenings.
  • Willingness for statewide travel.


Preferred Qualifications

Work Experience:
  • 6-12 months of experience in provider outreach.
  • 6-12 months experience providing services or conducting outreach across varied community settings.
  • Demonstrated ability to meet target goals/outcomes.


Skills
  • Strong oral communication skills including conducting presentations in a variety of contexts with a variety of audiences.
  • Excellent written and verbal skills.
  • Demonstrate ability to complete tasks on schedule while collaborating effectively with others.
  • Highly creative abilities in problem solving contexts.

Compensation Statement:

The pay range listed for this position is determined by our compensation program using market data and salary benchmarking. A combination of factors is considered in making compensation decisions including, but not limited to, education, experience and training, qualifications relative to the requirements of the position, and funding. At the University of Kansas Medical Center, a reasonable estimate for the starting pay range will be the minimum to midpoint of the posted range, taking into account the combination of factors listed above.
