Job Description

Purpose of Position:

Maintenance of daily operations within Jacobi's inpatient and outpatient pharmacies with latitude to exercise independent initiative and judgement. Functions as a clinical pharmacist to assigned patient care area and/or healthcare team member. Assist with institutional and departmental regulatory surveys as needed. Adhered to Federal State and local laws and other regulatory requirements regarding pharmacy and institutional practice.

Responsibilities:

Under direction, provides professional pharmacy services within Jacobi. Rotating between satellites, tours and shifts; performs all of the following tasks:

• Fills prescriptions, patient orders and monitor medication therapies. Resolves discrepancies in patient therapy. Follow-up on problems.
• Compound medications according to current USP guidelines
• Serves as a professional and technical resource for health system team, patients and families
• Ability to work cooperatively with health-system and pharmacy staff
• Utilizes clinical date in evaluating physician orders to ensure appropriate therapy, when necessary, acts accordingly.

Minimum Qualifications

Possession of a valid NY State license to practice pharmacy.
