Job Summary
We are seeking a compassionate and experienced Telephone Triage Registered Nurse (RN) to join our team in Irwindale, CA. In this role, you will provide telephonic nursing assessments and care coordination to patients during the night shift. Your clinical expertise and strong communication skills will help guide patients to the appropriate level of care, offering reassurance, education, and support remotely.
Key Responsibilities
Conduct telephone triage using clinical protocols to assess patient symptoms and guide care
Document all calls and nursing assessments accurately in the electronic health record (EHR)
Provide health education, guidance, and follow-up instructions based on assessment findings
Collaborate with physicians, care teams, and other health professionals
Escalate urgent medical issues to the appropriate on-call providers
Monitor and respond to messages and alerts as per protocols
Maintain knowledge of community resources and referral processes
Qualifications
Valid and active RN license in the state of California
Minimum 1 year of clinical RN experience (telehealth or triage preferred)
Strong critical thinking, clinical judgment, and decision-making skills
Excellent verbal communication and interpersonal abilities
Comfortable working independently in a fast-paced, remote environment
Proficient in electronic medical records (EMR) systems and general computer use
Ability to work 12-hour night shifts
