The hospice/home care educator is responsible for the assessment, development, implementation and ongoing evaluation of education for home care and/or hospice with an emphasis on ensuring compliance with local, state and federal regulations. Works in collaboration with the hospice and/or home care managers to ensure orientation, competency development and integration of new colleagues as well as the ongoing development of current staff. Additional responsibilities include serving as the informatics liaison and providing education regarding competence with the multiple types of technologies (computer, electronic health record (HCHB), learning management system, etc.) that are required for each role. The educator also functions as the liaison with schools, colleges and universities to coordinate student learning activities.

General Requirements:
- Orientation and Integration of New Colleagues
- Collaborates with orientee, information technology, preceptor, clinical manager, scheduler to develop and implement an individualized orientation schedule and learning plan.
- Collaborates with orientee, preceptor and clinical manager to develop an action plan and or re-education plan for unmet learning needs during the orientation process to ensure success.
- Provides oversight and supervision of field orientation.
- Reviews documentation and provides ongoing feedback to orientee regarding opportunities for improvement
- Ensures all orientation teaching materials are current and relevant
- Utilizes diverse teaching strategies during the orientation process to engage learner
- Function as a coach and mentor to new colleagues to ensure their learning needs are being met
- Assessment, Development, Implementation and Ongoing Evaluation of Educational Needs of Mercy One
- Participates in the development and implementation of education to ensure regulatory and 2 ongoing learning needs are met.
- Maintains a working knowledge of applicable Federal, State and local laws and regulations, the Integrity and Compliance Program, Code of Ethics, as well as agency policies and procedures and adheres in a manner that reflects honest, ethical and professional behavior.
- Coordinates monthly and ongoing education requirements for Home Health Aides.
- Collaborates with directors, managers, clinical quality review specialist, and quality management specialists to assess colleague competency and develops and implements effective education/training strategies to ensure ongoing competence.
- Collaborates with directors and managers to develop an action and/or re-education plan with specific target dates for unmet learning needs and/or competencies.
- Responsible for coordination and maintenance of education record keeping.
- Actively participants in all survey preparation initiatives to ensure compliance and maintain survey readiness.
- Assists with colleague clinical education needs and observation of clinical practice as requested by Clinical Manager or other appropriate leadership.
- Designs, implements, provides, and evaluates training to professional and nonprofessional staff as needed to meet departmental needs.
- Represents education at weekly process meetings and monthly team meetings for home care and/or hospice.
- Utilizes adult learning principles and diverse teaching strategies to meet the learning needs of colleagues.
- Maintains current understanding of clinician-based technology and incorporates ongoing review as a core responsibility.
- Educates, assists and supports clinicians with documentation in the electronic health record
- Collaborates with the manager to develop an education plan regarding clinical documentation issues not in keeping with hospice standards, when applicable.
- Keeps current in area of expertise and assures that standards and practices within the agency reflect best practice within area of expertise
- Performs duties on a consistent basis and in a timely manner.
- Utilizes multiple and effective communication strategies.
- Supports Patient Centered Care initiative.
- Coaches and mentor's colleagues to facilitate competence and enhance their individual growth.

- Collaborates with directors and managers regarding appropriate placement of students.

- Responsible for the ongoing supervision of all student affiliations to ensure achievement of learning objectives.
- Ensures all faculty and students meet federal, state and local regulatory body requirements.
- Contributes to the development of policies and procedures related to student clinical affiliations.
- Oversees any requests for an observational experience and ensures compliance with confidentiality statement
- Patient and Community Education
- Participates as needed in the development of educational materials for patients and the community.
- Utilizes principles of adult learning and age specific modifications in the development of education materials.
- Maintains awareness of hospice association resources, regulatory updates and trends and updates all educational resources and instruction accordingly, when applicable.
- Participates in the Development of Programs, In-services, Policies & Procedures, Committees, Continuing Education, and Competencies.
- Serves on committees as assigned.
- Contributes to the development of policies and procedures.
- Contributes to the development of competencies.
- Contributes to continuing education activities.
- Participates in special projects.
- Acts as a liaison with vendors as needed

Education:
- Graduate of an approved Nursing education program, bachelor's in nursing is preferred.
- Master's Degree in OT or ST or a Doctorate in Physical Therapy may be considered (Home Care only)
- Certification in OASIS within one year (Home Care only)
- Ten years of nursing practice including five in Hospice is preferred. Related experience in endof-life care, home health or oncology is a plus (Hospice only).
- Certification in Hospice and Palliative Care is required within one year of employment (Hospice only)
- CPR Instructor Certification preferred.
- Must be proficient in the use of Microsoft applications for creation of instructional documents, course curriculums, orientation materials and related visual aides
- Must possess accurate and clear oral & written communication skills
- Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program. In CA: Must have one year of professional nursing experience by regulation.
- Mental and Physical Requirements
- Frequently lift, stands, and walks (80% of time)
- Occasionally bend, stoop, and twist (10% of time).
- Occasionally position and lift patients (10% of time).
- Must be able to travel to various TH/THHS worksites or various work locations as required
