Job Title: Registered Nurse - Adult Care
Location: Manhattan, NY (10004)
Schedule: Monday to Friday, 9:00 AM - 5:00 PM
Hours: 40 hours/week (8 hours/day)
Pay Rate: $65 - $68 per hour

Position Summary:

We are seeking a dedicated and experienced Registered Nurse (RN) to provide high-quality adult home health care services in Manhattan, NY. This position is ideal for a compassionate professional with a background in acute care and current experience in adult home health. You will play a key role in delivering individualized care and supporting positive patient outcomes in a fast-paced, community-based setting.
Key Responsibilities:

• Provide direct, in-home nursing care for adult patients in accordance with individualized care plans.
• Conduct thorough patient assessments and monitor vital signs.
• Administer medications, IV therapies, and wound care when needed.
• Educate patients and caregivers on treatment plans, medications, and preventive care.
• Collaborate with physicians, case managers, and the interdisciplinary healthcare team.
• Document all care provided using electronic health records (EHR) in a timely and accurate manner.
• Ensure compliance with home health regulations and internal policies.

Qualifications:

• Active Registered Nurse (RN) license in New York State (Primary Source Verified)
• Current BLS certification (American Heart Association)
• Minimum of 2 years of acute care experience in one or more of the following:
  
  • Medical-Surgical (Med-Surg)
  • Maternal and Child Care
  • Critical Care
  • Pediatrics
• Recent experience in adult home health care is required.
