Job Title: Clinical Primary Therapist
Profession: LMSW or LCSW

Description:
Outpatient behavioral health background is required.
Must be familiar with New York State requirements and have prior experience working in New York.
On-call requirements are part of the position for off shifts and weekends.
The Clinical Primary Therapist works with clients individually or in groups to address mental health concerns.
The therapist applies knowledge and skills to assist clients in recovery and illness management.
Responsibilities include establishing the boundaries of therapy to create a comfortable and safe environment.
The therapist fosters a caring therapeutic culture and foundation for client and family engagement in recovery and behavioral management.
A comprehensive assessment is completed to gather data and information resulting in clinical formulation for the treatment plan.
In collaboration with the client and family, the therapist establishes treatment goals and identifies interventions to promote goal achievement.
The Clinical Primary Therapist must ensure timely and accurate documentation in the client's medical record.
Coordination of care with colleagues in the clinic and throughout the continuum is essential to secure appropriate treatment resources.
The therapist participates in on-call rotations to provide mental health crisis evaluations.
Comprehensive assessment includes psychosocial history, risk of harm, behavioral observations, substance use, and the client's chief complaint.
Diagnosis and potential problem statements are derived from assessment data.
Treatment plans must be person-centered, recovery-oriented, and responsive to the client's cultural and linguistic needs.
Goals and objectives in the treatment plan are clearly linked to the assessment and discharge criteria.
Treatment steps are attainable and include target dates and evidence-based interventions.
Treatment plans must be developed and signed by both the client and therapist within 30 days of admission.
Discharge criteria are outlined in the treatment plan, updating as needed to reflect client accomplishments.
The therapist completes and maintains clinic records and progress notes according to regulations and timelines.
Progress notes must summarize services provided and client responses relative to goals.
New information impacting treatment should be included in documentation and considered during treatment plan updates.
Counseling sessions should promote personal and social integration while providing health education on coping, medication, and symptom management.
Quality and effectiveness of therapy practice are systematically evaluated for both individual and group therapy.
Participates in quality improvement activities to initiate changes based on evidence-based interventions.
