Position Summary:
As a Primary Care Provider, you'll manage your own patient panel, providing high-quality, compassionate care through diagnosing and managing acute and chronic conditions with a focus on health promotion and disease prevention. Responsibilities include ordering tests, interpreting results, and working collaboratively within an interdisciplinary team.
Key Responsibilities:
- Conduct 80 patient visits per week within a 40-hour schedule.
- Take comprehensive health histories and perform focused physical exams.
- Diagnose, treat, and communicate effectively with patients about clinical findings.
- Formulate and implement care plans based on best practices.
- Collaborate with a team to ensure quality and patient-centered care.
Requirements:
- Accredited APN Program completion and Massachusetts licensure as a CNP.
- BLS certification (bilingual skills preferred).
- 2 years of experience as a Primary Care Nurse Practitioner or completion of a Family Medicine Nurse Practitioner Residency, ideally in a community health setting.
