Salary Range: $33.2400 - $51.3800 /hour. Actual compensation may vary based on geographic location, work experience, skill level, and education.
Job Summary
The Population Health Coordinator implements programs and projects to promote and grow a specific service line (i.e. record chart abstractions, HPP learning education sessions, marketing material distribution.). This role handles scheduling within Hoag and with network providers, compiles data and reports, evaluates participation/satisfaction of programs, assists in health plan program performance, and other administrative aspects of projects. Educates and communicates activities, programs, and service offerings to patients, providers and staff to best fit program needs. Articulates and tailors information to clinical teams, physicians, and/or staff, answers questions and develops resources as needed. Analyzes participation data and assesses need for development of other program components for improved performance. Plans and prepares events e.g. health plan meetings, community outreach events, and conferences. Performs other duties as assigned.
Education and Experience
Bachelor's Degree in Communications, Marketing, Exercise Science, Nutrition, Nursing or 4 years' of health care experience with an Associate's Degree in related field or licensed candidate required. Master's in public health preferred.
