Scope of Position: The Assertive Community Treatment (ACT) team provides mobile community based mental health treatment to promote the recovery and community integration of individuals with serious mental illnesses, and high rates of co-occurring substance use disorders. The team specifically serves individuals who are residing in the New York City Department of Homeless Services mental health shelters in The Bronx and Manhattan.

Essential Position Functions:
- Oversee the clinical and administrative operations of a multi-disciplinary treatment team, and report all activities to the Assistant Vice President of Community Support Programs.
- Direct clinical practice and supervise the intake process.
- Conduct clinical assessments and develop individualized treatment plans.
- Provide regular supervision to all program staff.
- Ensure the team meets performance outcomes as defined by New York State Office of Mental Health (OMH) licensing standards and New York City Department of Health and Mental Hygiene (DOHMH) contracting standards.
- Oversee incident reporting procedures and ensure adherence to The Bridge IRC policies and procedures, including compliance with IRC recommendations and guidance.
- Coordinate Assisted Outpatient Treatment (AOT) reporting and communication, ensuring 100% compliance.
- Lead daily organizational meetings and weekly service planning meetings.
- Train, supervise, and evaluate Assertive Community Treatment (ACT) staff.
- Conduct utilization reviews and quality assurance assessments to ensure ACT team operations comply with OMH guidelines and DOHMH operating standards.
- Supervise electronic chart documentation in accordance with confidentiality regulations.
- Provide in-service training and support staff development to ensure competency in ACT core practices and community-based supervision strategies.
- Promote the integration of risk assessment protocols and evidence-based interventions aimed at reducing recidivism and preventing harmful outcomes, including arrests, incarceration, substance use, public disturbances, self-harm, emergency room visits, and hospitalizations.
- Oversee, coordinate, and provide 24-hour on-call crisis consultation.
- Ensure compliance with CAIRS reporting and other required reporting by OMH and DOHMH.
- Facilitate collaboration and reporting with Health Homes.
- Coordinate Medicaid billing through electronic medical records by identifying eligible, billable progress notes, and ensure the team meets billing targets as defined by the program budget.
- Ensure all eligible consumers have active Medicaid coverage.
- Collaborate with medical staff to oversee medication management systems, including billing, storage, and ordering.
- Guide the team in the effective use of service dollars to support consumers' service plan goals and needs.
- Authorize time sheets, supply orders, vacation requests, and other administrative functions to ensure the smooth operation of the team.
- Provide culturally competent services in accordance with The Bridge's policies and procedures.
- Perform other duties as assigned by Co-Directors and executive leadership.
Requirements
- Master's degree or higher in Social Work, Psychology, or Mental Health Counseling.
- Current professional licensure in one of the above fields is required.
- Minimum of three years of experience working in mental health or substance use services, preferably with an ACT (Assertive Community Treatment) team.
- Supervisory experience is required.
- Candidates without a professional license will not be considered.
- Spanish-speaking candidates are highly desirable.
