Key Responsibilities:
- Perform comprehensive physical assessments to develop personalized therapy plans.
- Create and implement progressive therapy programs to address mobility, pain, balance, and functional limitations.
- Provide hands-on treatments, therapeutic exercise, gait training, and fall prevention strategies.
- Help residents safely regain and maintain autonomy in daily activities within the assisted living setting.
- Educate residents, families, and facility staff on exercises, care strategies, and wellness techniques.
- Maintain detailed records that meet healthcare standards and support continuity of care.
- Partner with nurses, caregivers, and other therapy staff to ensure coordinated, whole-person care.
- Monitor patient progress, adjust treatment plans as necessary, and report any changes in the patient's condition to the physician and family.
Requirements
- Active New Jersey Physical Therapist license (required)
- Doctorate or Master's Degree in Physical Therapy from an accredited program
- At least 1 year of PT experience; experience with geriatric or assisted living populations strongly preferred
- Strong clinical knowledge, empathetic communication, adaptability, and teamwork
- CPR and First Aid certified (preferred). Additional certifications in geriatrics or neurological rehabilitation (a plus).
- Valid driver's license and reliable transportation (required for travel to patients' homes).
