Job Title: Outpatient/Inpatient Social Worker/RN Case Manager
Profession: Nursing

Experience: Minimum of 3 years in a clinical care setting
License: Valid, current, full, and unrestricted Professional License to practice LMSW or LCSW in any state in the USA
Certifications: Current Basic Life Support (BLS) certification
Must-Have:
- Must have and maintain a current Basic Life Support (BLS) certification by the American Heart Association throughout employment.
- Licensed Master Social Worker (LMSW).
Description:
The incumbent serves as an Outpatient/Inpatient Social Worker/RN Case Manager.
This role is responsible for direct supervision and supportive contact for a specific group of identified high-risk patients.
The Social Worker/RN Case Manager is responsible for designing and managing a continuum of care.
The focus is on empowering clients to achieve demonstrable outcomes and self-sufficiency.
The Social Worker/RN Case Manager is responsible for assessment, service planning, and resource acquisition.
Monitoring progress and responding to emerging client needs is essential.
The position links clients with community social service providers, healthcare providers, substance abuse, and mental health providers to achieve specific goals.
The incumbent will perform utilization review of patient cases in both inpatient and outpatient services.
This includes review of medical necessity for referrals to specialty providers for services not typically provided within the facility.
Collaboration with a multidisciplinary team is important to promote the efficient use of a variety of healthcare resources.
The role aims to provide quality care, decrease fragmentation of care, enhance patient self-care and quality of life, and achieve cost containment.
The incumbent will work closely with providers, nurses, social workers, physicians, and all healthcare staff to ensure implementation of appropriate resources.
Responsibility for fulfilling utilization goals for the hospital and third-party payors is necessary.
The Social Worker/RN Case Manager reports to the Director of Case Management/Care Coordination.
Experience and knowledge of discharge planning, case management, or utilization review processes is needed.
Demonstrated knowledge of electronic health record systems is required.
Orientation occurs every two weeks.
The organization does not accept religious exemptions for COVID-19 vaccination.
If a medical exemption is requested, approval must be obtained from the medical review board prior to onboarding.
Housing applications must be submitted two weeks prior to the start date.
Holiday coverage is required.
