Stroke Care Nurse Practitioner
We are seeking a dedicated and skilled Stroke Care Nurse Practitioner to join our full-service healthcare facility located in Modesto, California. As a pivotal member of our stroke care team, you will provide expert nursing consultation and oversight, acting as a bridge among medical staff, nursing staff, and support services. This role involves coordinating multidisciplinary care based on evidence-based practices, delivering exceptional stroke education, and supervising patient care throughout all stages, including post-discharge. You will collaborate closely with key stakeholders within the Stroke Program to uphold and enhance certification requirements through ongoing review of data and performance improvement projects. This position is integral to incorporating the patient experience into all aspects of care, ensuring the highest standards of health and well-being for our community.
Salary: $140,000.00 - $200,000.00 per year

Essential Duties and Responsibilities:
- Provide expert nursing consultation and practice oversight within the stroke program.
- Function as a liaison between medical staff, nursing staff, family members, and support services.
- Coordinate multidisciplinary care based on the latest evidence-based practices.
- Responsible for the delivery and oversight of comprehensive stroke education and patient care programs.
- Facilitate ongoing communication and collaboration with Stroke Program key stakeholders.
- Participate in the review and analysis of program data to drive performance improvement initiatives.
- Incorporate patient feedback and experiences to continually enhance the quality of care provided.
- Ensure compliance with stroke program certification requirements.
Minimum Position Qualifications:
- Active California RN license.
- State of California Nurse Practitioner licensure with a furnishing number.
- DEA furnishing certificate.
- Current Basic Life Support (BLS) certification.
- Advanced Cardiovascular Life Support (ACLS) certification.
- NIHSS certification required within 3 months of hire/transfer.
- ASLS certification required within 3 months of hire/transfer.
- ENLS certification required within 6 months of hire/transfer.
- Must possess either a CNRN or NVRN certification.
- Master's Degree in Nursing or a related field.
Preferred Qualifications:
- Certification as a Stroke Registered Nurse (SCRN) is highly preferred.
