The Early Childhood Education and Care Department was established on July 1, 2020. The Department's aim is to create a more cohesive, equitable, and effective early childhood system in New Mexico. That means coordinating a continuum of programs from prenatal to five and ensuring that families in every corner of the state can access the services they need.

Why does the job exist?

The purpose of this Registered Nurse III role is to support the ECECD vision: A culture where all New Mexican families and young children are thriving. This is done through a home visiting model that focuses on the immediate postpartum period. The Home Visiting Nurse conducts home visits with families with newborns in their assigned area. This nursing role, will participate in a community-based program called Family Connects that supports new parents in caring for their newborn(s), offers physical assessments of the mother and the baby, addresses questions about caring for the newborn(s), identifies parents' needs, and helps to identify community services or resources to meet those needs. The Home Visiting nurse will work with team to schedule families for an initial Family Connects home visit in which the nurse will conduct an Integrated Home Visit between two to twelve weeks of birth ideally targeting the third week postpartum.
The goal is to support and assess family needs in 12 domains of physical and psychosocial well-being and facilitate referrals and connections to community agencies to support families in these areas. The home visiting nurse will work in a team environment of clinical and non-clinical staff in order to schedule, visit, and support families in the community using the evidence-based Family Connects NM home visiting protocol. In addition, the home visiting nurse will help caregiver(s)/parent(s) understand that having a newborn is a universal time of need; the variable is the amount of need each family has. The FCNM team continuously works to de-stigmatize home visiting, a notion that is critical to the success of a universal approach in supporting all families with a newborn.

How does it get done?

The Family Connects Home Visiting Nurse will follow the Family Connects home-visiting protocol, keeping detailed records of client observations and activities. They will document family assessments and program evaluations in real time using a computerized record system at home. The nurse will adhere to the Family Connects scheduling protocol and visit target area hospitals as needed to schedule home visits for new mothers.
They will conduct health and physical assessments of both mother and baby in the home to determine their well-being. This includes assessing family needs across 12 domains of physical and psychosocial well-being, such as depression, substance abuse, and domestic violence. The nurse will use clinical skills to appraise situations and make informed decisions, establishing a trusting relationship with families during in-home visits. They will work with families to create a support plan with goals, objectives, and activities that address family strengths and needs.
The nurse will collaborate with a team to ensure appropriate referrals and connections to community resources, assist parents in making and attending appointments with doctors and social service agencies, and act as a liaison between families and other community agencies. They will apply their knowledge of parent-child interaction, maternal and child health, child development, and child abuse/neglect to help families improve parenting skills, increase supports, reduce stress, and address parental needs. The nurse will assist in creating a health plan for the entire family.
They will maintain eight open visit slots and conduct and document at least six home visits per week, responding to client needs through flexible scheduling, including occasional evening or weekend visits. The nurse will apply effective time-management, organizational, and multitasking skills, with documentation completed in compliance with state guidelines, ideally within 24 hours of the encounter.
As an active member of the Family Connects team, the nurse will attend weekly team meetings, clinical supervision, staff meetings, and other required meetings. They will support and encourage the development of other staff, and the Family Connects program.

Ideal Candidate

The ideal candidate for this position must possess and maintain a current New Mexico Registered Nursing license or eligible for licensure in accordance with Nursing practice Act 61-3-2 through 61-3-+31 NMSA 1978 and 16-1-1 through 61-12-20 NMAC, as applicable.
Referred skills include (a) Spanish language proficiency, (b) clinical experience related to domestic violence, substance abuse and/or depression, (c) one or more years of experience in child/maternal health, social work, or other related field, and (e) Certified Lactation Educator or Consultant. Ability to work in a team with clinical and non-clinical staff. Ability to work with nurse supervisor to implement a practice improvement and clinical development plans. Ability to work on weekends, as required Proficient and comfortable using laptops as part of in-home assessment and intervention. Must maintain current CPR certification.

Minimum Qualification

Must possess a current license to practice as a Registered Nurse in New Mexico, or eligibility for such licensure in accordance with New Mexico requirements, or eligibility for a temporary license issued for employment at a specific institution in New Mexico and three (3) years of experience as it pertains to the essential duties and responsibilities of the classification.

Employment Requirements

Must possess and maintain a valid Driver's License. Pre-employment background investigation is required and is conditional pending results. Defensive Driving Certification must be obtained within the first month of hire.

Working Conditions

Services are provided primarily provided in the family home, but can also include clinics, auxiliary and community sites. Work is performed in an office setting with exposure to Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage with extended periods of office noise, bending, standing and lifting. Ability to lift up to 50 lbs. Services are provided in a clinic with diverse racial and ethnic populations, many who do not speak the English language. Occasional work outside of normal work schedule to attend training and community initiatives. Travel required.
