Job Title: LTC/SNF RN
Profession: Registered Nurse

Shift: Various (8 and 12-hour shifts)
Hours per Shift: Typically 8 hours, with some 12-hour shifts
Experience: Minimum of 2 years
License: Required
Certifications: BLS
Must-Have: BSN Required
Description:
This position is for a Registered Nurse in a long term care and skilled nursing facility.
The facility has 28 beds with private rooms and a typical census of 23-28 residents.
Common diagnoses include Dementia, COPD, CHF, DM, HTN, and more.
The role involves initial assessments, medication passing, and charge nurse responsibilities overseeing nursing assistants.
Skin assessments, pain assessments, admissions, wound care, and pressure ulcer prevention and treatment are also part of the job duties.
Family involvement and communication are essential in this role.
Mechanical total lift systems are used for resident transfers.
The EMR system currently in use is Meditech, with a transition planned to a new system in June 2024.
Staffing ratios are approximately 1 nurse to 23-28 residents.
RNs are not required to attend team conferences for acute inpatient rehab.
The interdisciplinary team includes physical therapy and occupational therapy, with daily meetings.
There is a pharmacy on-site that provides bubble packs.
Schedule requirements include day shifts from 0600-1430 and evening shifts from 1415-2245.
Nights may be from 2230-0700, with possible rotation between different shifts to meet staffing needs.
Weekend requirements are every other weekend, and holiday rotation may be necessary.
There are no mandatory call or standby requirements, and floating is not required.
Scrubs may be any color.
Department-specific orientation includes 1/2 day general orientation, followed by 1-2 days with an educator and 1-3 days with a preceptor on the designated shift.
