In this unique and rewarding role, you will help patients improve mobility,manage pain, and maintain independence-while also supporting comfort andquality of life for those in hospice. The Physical Therapist is solelyresponsible for evaluating and diagnosing a patient, preparing a treatment planand therapeutic goals, including determining number and frequency of visitsproviding or supervising all care, and determining appropriate discharge of apatient. Youwill provide continuing care through these responsibilities

Essential functions of this position include the following.

• Perform comprehensive assessments and develop individualized careplans based on physical, emotional, and functional needs
• Assess and evaluate therapeutic, rehabilitative and/or functionalstatus; participate in the development of the total plan of care
• Assess for muscle strength, mobility gait, ROM-potential forrehab
• Determine priority needs for therapy patients
• Initiate Physical Therapy program and instructs other personneland/or family/caregiver members in certain phases of physical therapy withwhich they may work with the patient, as well as instructing them as to thegoals of the Physical Therapy program for the patient by participating in caseconferences
• Evaluate the home environment for safety
• Deliver physical therapy services in patient homes, focused onrestoring function, mobility, and quality of life
• Educate patients, families, and caregivers on therapy goals,exercises, and techniques for safety and independence
• Collaborate closely with nurses, social workers, chaplains,physicians, and other team members (as needed)
• Monitor and document patient progress, revising care plans asneeded; participate in discharge planning
• May train patient in the use of prosthetic and/or orthoticdevices
• Strictly follows infection control procedures and safety policies
• Adequately supervise patient care given by assistive personnelsuch as Physical Therapy Assistants (PTAs) in accordance with regulations andcompany leadership principles
• Maintain compliance with all federal, state, and organizationalpolicies, including those of the Washington State Department of Health
• Participate in staff meetings, training, and quality improvementinitiatives
• Participate in continuing education to stay informed of trends,practice developments and maintain licensing requirements

Our requirements and qualifications for success

• Graduate of an accredited Physical Therapy program (DPT or MPT)
• Current Washington State Physical Therapist license in goodstanding
• 2-5 years' of experience
• Current CPR & BLS certification
• Experience in Home Health and/or Hospice is highly valued
• Reliable transportation, valid driver's license, and proof of autoinsurance
• Basic proficiency with electronic medical record systems (EMR)
• Compassionate, patient-centered care mindset
• Strong clinical judgment and ability to make ethical decisions
• Ability to work independently and collaboratively
• Excellent communication, prioritization and time management skills
• Respectful and sensitive to patient cultural, spiritual, andemotional needs

Physical,environmental, and mental requirements

• The ability to listen to, understand and communicate informationand ideas presented through spoken words and sentences
• The ability to tell when something is wrong or is likely to gowrong. It does not involve solving the problem, only recognizing that there isa problem
• The ability to use your abdominal and lower back muscles tosupport part of the body repeatedly or continuously over time without"giving out" or fatiguing
• The ability to lift up to 50 lbs
• The ability to function in other people's home setting which canvary widely with the course of a day or week
• Job requires being sensitive to others' needs and feelings andbeing understanding and helpful on the job
• Job requires being reliable, responsible, and dependable, andfulfilling obligations

SalaryRange: $108,200 to $135,200 per year
