We are seeking a dedicated and experienced Physical Therapist to join a collaborative Occupational Health & Wellness team in Merrillville, IN. This role focuses on rehabilitative care and injury prevention in an outpatient setting, particularly geared toward work-related musculoskeletal conditions and orthopedic concerns.

Schedule:
- Monday & Wednesday: 8:00 AM - 5:00 PM
- Tuesday: 9:00 AM - 7:00 PM
- Thursday: 9:00 AM - 5:00 PM
- Friday: 8:00 AM - 4:30 PM
We are seeking a dedicated and experienced Physical Therapist to join a collaborative Occupational Health & Wellness team in Merrillville, IN. This role focuses on rehabilitative care and injury prevention in an outpatient setting, particularly geared toward work-related musculoskeletal conditions and orthopedic concerns.
Key Responsibilities:
- Assess and diagnose functional movement impairments and workplace-related injuries
- Create and implement personalized treatment plans to support mobility, recovery, and performance
- Focus on injury prevention strategies and long-term wellness initiatives
- Perform orthopedic and manual therapy techniques tailored to patient needs
- Track patient progress, set clinical goals, and adjust therapy plans to optimize results
- Collaborate with a multidisciplinary health team to promote holistic recovery
- Educate patients on ergonomics, posture, fitness, and home exercise to prevent reinjury
- Ensure documentation and care delivery aligns with regulatory and professional standards
- Minimum 2 years of outpatient or general orthopedic physical therapy experience
- Active Physical Therapist (PT) License - Indiana
- Strong interpersonal and patient education skills
- Familiarity with workplace health and injury prevention programs preferred
