Job Summary
The Population Health Manager will assist in improving health outcomes for CVCH patients by leading CVCH in successfully developing, implementing, and monitoring initiatives to meet requirements for value-based care incentive programs. Under the direction of the Chief Quality and Compliance Officer, they will ensure success in efforts to work with payers and Accountable Care Organizations (ACOs) in managing incentive plan goals and reporting, engage physicians and staff to facilitate completion of incentive-based care, manage value-based incentive program patient enrollment and rosters, develop and implement systems to engage patients in their healthcare through care coordination, care management, outreach, case management and enrollment, and direct efforts in managing identified chronic care or resource intensive patients. They will provide direct daily supervision of the Population Health Department.

a. Ensures monitoring, analysis, management, and reporting out on clinical quality metrics
b. Establishes measures to monitor clinical quality
c. Ensures education of clinical staff on quality metrics
4. Supervisory Skills
a. Performs various administrative supervisory functions including monitoring and evaluating staff performance, establishing work priorities and coordinating schedules, maintaining personnel and payroll records and interviewing, hiring and training staff, and implementing corrective action when appropriate

Job Specifications
Education: Bachelor's Degree in Business Administration, Healthcare management, or health related field. Masters preferred. Experience may be substituted for education. Licenses/Certificates/Registration: Current Washington State driver's license and proof of automobile liability insurance coverage. Healthcare Financial Management Association (HFMA) managed care certification preferred. Experience: Five (5) years of experience in analysis and coordination of Population Health related strategy, operations, or financial management, or two (2) years of experience in Healthcare Administration, overseeing staff, managing patient services, or other leadership in health center administration, with strong understanding of quality/value-based/population health concepts. Language Skills: English required. Essential Technical/Motor Skills: Knowledge of and skills in contract negotiations and contract legal review. Strong skills in analytical/financial analysis. Proficient in Excel, Word, and other software programs. Ability to build and maintain effective relationships within the organization and with payors, internal partners, and physicians. Ability to leverage interpersonal skills and technical knowledge to bring people and ideas together to perform effectively. Ability to influence others to achieve understanding, acceptance, and commitments to act in support of ideas, programs and products.
