Essential Duties and Responsibilities:
- Supervise day-to-day clinical operations and ensure continuity of care in the absence of staff.
- Conduct on-site supervision and provide direct oversight to clinical staff, ensuring adherence to quality standards and patient care protocols.
- Proactively address operational and clinical challenges to prevent patient harm and enhance organizational efficiency.
- Ensure comprehensive execution and reassessment of patient care plans according to significant health status changes or post-hospitalization.
- Develop policies and advocate for procedures based on industry regulations and evidence-based practices.
- Maintain compliance with legal and regulatory standards, assuring team performance aligns with agency expectations.
- Facilitate case conferences to discuss patient care, plan of care adjustments, and discharge planning.
- Delegate clinical tasks in alignment with staff credentials and patient needs, optimizing clinical outcomes.
- Participate in the recruitment, retention, and training of qualified clinical personnel.
- Manage staff schedules, approve payrolls, and ensure fulfillment of productivity standards and organizational goals.
- Lead educational seminars to foster staff development and adherence to current healthcare practices.
- Engage in community awareness programs to bolster the organization's public health mission.
Minimum Position Qualifications:
- BSN: Bachelor's Degree in Nursing required.
- Current, valid RN license issued by NYS Department of Education.
- Demonstrated experience in Medicare home health settings and administrative roles in clinical management.
- Strong organizational, prioritize effectively multiple critical tasks.
- Excellent communication skills, both oral and written, capable of engaging various stakeholders.
- Valid NYS driver's license and reliable transportation for any weather condition.
- Adept at navigating change and workplace interruptions with professionalism.
Preferred Qualifications:
- Significant experience in Home Health Care Agency operations, preferably at a case management level.
- Prior role in leadership positions within a clinical setting, managing cross-functional healthcare teams.
