Job Description

SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:
Under direction of the Director of Social Work Services will provide oversight of services delivered by the team and demonstrates proficiency in guiding the all phases of the intervention. This role demands a dynamic and forward-thinking professional with a strong commitment to phased based treatment, collaboration with multiple stakeholders and delivering high quality services to the systems most vulnerable patients. The ideal candidate will possess exceptional leadership skills, a strategic mindset, and the ability to drive impactful change across the services spectrum. This position requires a blend of clinical expertise, leadership skills, and administrative acumen to ensure the efficient functioning and the provision of high-quality care.

QUALIFICATIONS FOR THE JOB: (In addition to the minimum qualifications as per the corporate job description)
Clinical Oversight
  • Responsible for the clinical and administrative supervision of all CTI Team members.
  • Ensures CTI teams' practice is consistent with the fidelity to the CTI model; adherence to phase transition dates and caseloads are followed through.
  • Leads daily team/organizational meetings and weekly service planning meetings.
  • Ensures that model-specific case planning and recording documents are being completed correctly and are up to date for all CTI workers
  • Forge and maintain collaborative relationships with healthcare providers, community organizations, government agencies, and advocacy groups to address clinical care challenges, social determinants of health, and promote health equity.
  • Allocates time to both clinical and administrative duties, as indicated.

  • Administrative Responsibilities
  • Coordinates team performance with the Central Office lead of CTI.
  • Acts as a liaison to the Office of Mental Health and any other regulatory agencies.
  • Responsible for Performance Improvement projects, Utilization Reviews and Incident Management activities for the program.
  • Oversees and manages Electronic Health Record (EPIC) demands, leveraging EMR functionality to enhance practice and streamline operations.
  • Collaborate closely with facility-based leadership or designated champions to drive and manage special initiatives that align with organizational objectives.
  • Prepares reports for funders, stakeholders, and regulatory bodies to demonstrate program effectiveness and accountability.
  • Completes internal fiscal reports as advanced by our Finance Teams. Responsible for Patient Service Dollars.

  • Managerial Responsibilities
  • Oversees time and leave responsibilities of the CTI team members.
  • Upholds high ethical standards and ensuring that all program activities are conducted with integrity and respect.
  • Develops and implements plans to address crises or unexpected challenges that may affect the program or its clients.
  • Promotes the program and raises awareness about the services offered.
  • Advocates for policy changes or additional resources to better serve the target population.

  • ALL ABOVE DUTIES MAY CHANGE AT THE DISCRETION OF THE DEPARTMENT HEAD/SUPERVISOR, ACCORDING TO THE NEEDS OF THE SERVICES.

    Minimum Qualifications
    1. Valid license as an LCSW issued by the NYSED, and two (2) years of supervisory experience, as described above.
